Completed Address Change form or letter. Request must include the owner signature.

Name change

Marriage, divorce, etc.

  • Marriage Certificate, Certified Divorce Decree reinstating maiden name, or other official documentation affecting name change, i.e. Certified Court Order.

Company or Corporation

  • Certificate of Name Change including Tax Identification Number.

Merger of Company or Corporation

  • Certificate of Merger including Tax Identification Number.

Appointment of AIF (Attorney in Fact)

  • Copy of AIF / Power of Attorney.
Conveyance

Copy of Assignment / Conveyance filed in the County / Parish where the property is located.

Divorce

Recorded copy of final Divorce Decree, including Settlement Agreement, and, if appropriate, recorded Assignment / Conveyance.

Trust Partnership

Trust or Partnership is created

  • Trust or Partnership Agreement, and, if appropriate, recorded Assignment / Conveyance of property to a Trustee of a Trust or Partnership (conveyance must name the Trustee).

Trust or Partnership is terminated

  • Dissolution of Trust or Partnership document, and, if appropriate, recorded Assignment / Conveyance to beneficiary.

Change in Trustee

  • Copy of Trust Agreement, Memorandum of Trust, or relevant pages of the Trust Agreement (naming the Trust, Trustee, Successor Trustee, etc.) and copy of resignation of Trustee and acceptance of successor Trustee.
Estate Proceedings

Owner died Testate (with a will) and probate proceedings are being conducted in the state where the property is located.

  • Death Certificate, Order Admitting the Will to Probate in the County / Parish the property is located, Letters Testamentary, Evidence debts and taxes have been satisfied, Final Decree of Distribution or Judgement of Possession in the County / State where property is located.

Owner died Testate (with a will) and probate proceedings are being conducted in the state other than where the property is located.

  • Ancillary probate proceedings opened in the County / Parish where the property is located or, if monetary / acreage threshold is not exceeded, follow instructions for intestate succession (see below).

Owner died Intestate (without a will).

  • Death Certificate, Letters of Administration (if applicable), Affidavit of Heirship which has been signed by a disinterested 3rd party and recorded in the county / parish the property is located, evidence debts and taxes have been paid, and W-9s for heirs listed in the Affidavit of Heirship. State laws of Descent and Distribution will apply.
